President Cyril Ramaphosa signed the National Health Insurance (NHI) Bill into law on Wednesday, 15 May 2024, in a move aimed at revolutionising South Africa’s healthcare system. The new system is geared towards achieving universal health coverage and addressing socio-economic disparities. Despite this milestone, the full implementation of the legislation is still a process that could potentially take many years.
National Health Insurance is a health financing system designed to pool resources to offer health services to all South Africans based on their health needs, regardless of their socio-economic status. Benefits will be determined by available funding and the providers of services will be from both the public and private sectors. According to South African law, there is no provision for NHI funding, therefore, the Minister of Finance is yet to introduce the Money Bill in the National Assembly.

It is important to note that for the foreseeable future, both medical schemes and the private healthcare sector will operate as usual. Put simply, medical scheme members and healthcare providers throughout the system will not experience any immediate impact.

However, the complete implementation of the Act is still a long way off, and various legal challenges are currently in progress.
A comprehensive and sustainable funding model for the NHI in South Africa has not been developed yet. Given the country's constrained fiscal resources, sluggish economic growth, and a small tax revenue pool, it appears that a viable resolution can only materialise over an extended period. In its current form, it is difficult to envision a scenario where there will be sufficient funding to support a comprehensive NHI.
